is intended to assist in sequring such informat ion in more or less uniform outlines. We urge the addition on a
separate sheet, of such ot her important and in~eresting information a-s cannot be conveniently plaeed on this blank.
In the case of men now living, the information can, of course, be only up to date. Photographs of living and
deceased ministers, diaries, letters and documents of historical interest are earnestly solicited. Your co-operation
